لان اذا تذكرون اكو انزيم اسمه lipoprotein lipase موجود بـ adipose tissue من ماكو انسولين يتفعل ويكسر TG وتروح للـ liver بشكل FA بالكبد تتفعل عملية lipogenesis
ترجع تتحول TG ..
❔.....السؤال الثاني هو ليش بعض مرضى NAFLD ممكن مستقبلاً اذا ما امتثل للعلاج يتحول الى liver cirrhosis ❓
بكل بساطة بسبب الدهون المتراكمة راح تزداد oxidative stress او free radicals فتسوي inflammation ، فتتحول انسجة الكبد من انسجة سليمة الى انسجة صاير بيها fibrosis وهيج تصير بعض complication مثل hepatic encephalopathy وغيرها ..

جداً ضروري ويعتبر العلاج الاساسي

2⃣ Insulin sensitiser (pioglitazone)
15–30 mg once daily
بس لازم انتبه اذا الشخص كان عندة edema او HF

❕.....بالنسبـة للـ metformin ميعتبر علاج اساسي بس اكو دراسات تكول انُ اظهر تحسن على مستوى نقص انزيمات الكبد وكمية الدهون المتراكمة بس ما تم التأكد انه هو مفيد من ناحية منع تطور المرض.

3⃣Antioxidant (Vit E )

وبعض الاطباء يكتبون عشبة milk thistle مثل علاج legalon لان يحمي خلايا الكبد من التلف الناتج من
Free radicals

الطريقة الوحيدة حتى اشوف المرض بأي مرحلة حالياً هي اخذ biopsy و CT مو كافي ..

💠 خلي نوضح اعطاء الantibiotics اثناء الحمل بشكل مختصر
1️⃣Penicillins and Cephalosporins

➡️are the antibiotics of choice during pregnancy (category B).

may be used when resistance testing indicates that they are needed (C)

3️⃣Erythromycin, Clarithromycin, Azithromycin & Roxithromycin

may be used in when the resistance spectrum requires them, or in cases of an allergy to penicillin. Clarithromycin (category C) erythromycin estolate (D)

should only be used when Penicillins, Cephalosporins and Macrolides have failed (C)

are contraindicated during pregnancy (category D).

6️⃣ Co-trimoxazole & Fluoroquinolones

➡️are second choice during pregnancy (category C)

7️⃣ Aminoglycosides

➡️should only be used in life-threatening infections (All are category D, except Gentamicin is category (C).

8️⃣ Nitrofurantoin

➡️is commonly to treat UTls in pregnancy (category B), but not recommended after the 36th weeks of gestation (may cause hemolytic anemia in fetus)

9️⃣ Fosfomycin

➡️may be used to treat UTis in pregnancy (category B)

1️⃣0️⃣ Metronidazole

➡️(category B) is to be preferred, but some references not recommend it during first trimester.

💠select antihistamine for special population

1️⃣liver failure

Bilastine is the best choice (it is not metabolized in the liver).
Desloratadine, Fexofenadine and Levocetirizine are also NOT require dose adjustments.

2️⃣chronic kidney disease (CKD)

Bilastine are the best options for long term use
Desloratadine and Fexofenadine are require dose adjustments.
Avoid Levocetirizine in patient with CrCl
3️⃣HT and DM
Most antihistamines are safe

4️⃣epilepsy

وخاصة مع
Diphenhydramine and Cetirizine
لذلك فنفضل
Bilastine, Loratadine Desloratadine and Fexofenadine

5️⃣Elderly
الجيل الاول عدهم anticholinergics effect لذلك نحاول نبتعد عنهم

Confusion and memory loss in older people; increased risk of dementia in older adults
Constipation.
Urinary retention in people with Benign Prostatic Hyperplasia (BPH).
Glaucoma risk for acute angle-closure glaucoma in patient with closed angle glaucoma.

6️⃣ pregnancy

Loratadine and Cetirizine should be preferred as they are the best-investigated antihistamines.
If a sedative effect is desired, Clemastine, Chlorpheniramine and Diphenhydramine

7️⃣ lactating women

All antihistamines are considered safe to use during breastfeeding
Cyproheptadine should be avoided during lactation because it may inhibit Prolactin levels and decrease lactation.
